For a limited time, Dialogic is offering you the opportunity to purchase the latest Dialogic® Brooktrout® Fax Products - and receive a rebate of $50 to $100 (US) per port on the products you are replacing by trading them in to Dialogic.

Make your fax purchase between October 15, 2011 and December 31, 2011. Visit this registration page to input information about the products you purchased and the ones you're trading in on or after November 1, 2011. Your rebate is equal to $50 or $100 (US) per port on the traded-in products.

How Your Rebate Is Calculated

Calculating your rebate is simple. The products that you are purchasing are considered "Eligible Products", and the products that you are replacing and sending to Dialogic are called "Trade-In Products". A detailed list of Eligible Products and Trade-In Products is available here.

If the number of ports on the Eligible Product(s) in your order is equal to or greater than the number of ports on the Trade-In Product(s) that you are replacing and trading in to Dialogic, your rebate is US$100 multiplied by the number of ports on the Trade-In Product(s). You may receive rebates on multiple orders of Eligible Products that are accompanied by Trade-In Products, provided the port requirement is met each time.

You can also include one (1) or two (2) Dialogic® Media Gateway units (or bundles) with your Eligible Product purchase, in which case you will receive an additional rebate that is determined according to the rebate schedule in Appendix A of the Product List.

The Fax Upgrade Program begins on November 1, 2011 and ends on December 31, 2011, and the Program is available worldwide.

When you are ready, fill in the form on this page, starting November 1, 2011. You will need the model number and serial number for your Eligible Product(s) and your Trade-In Product(s).

If you wish to submit a Trade-In Product that is not among those listed, then before submitting your rebate request, contact us and provide the brand name, model, and description, including number of ports in the text field on the last screen. We will review your request and let you know if your proposed Trade-In Product(s) is/are acceptable.

Nothing within this Fax Upgrade Program description varies or modifies the official Terms and Conditions.

Important Information for Rebate Customers

• Your rebate(s) will not be processed without the correct serial numbers for the products you purchased and those you're trading in. Please double check that you have provided the correct serial numbers in the form below.
• Please do NOT submit multiple online forms. Add lines to list multiple products that were purchased and/or that are being traded in.
• You are only eligible for a rebate if the number of fax ports on the product(s) you purchased is greater than or equal to the number of fax ports on the product(s) you're trading in. Please review the Terms and Conditions for detailed information as to how your rebate is calculated.
• Only fax ports on returned board(s) which are in working condition will be considered eligible towards a rebate. Non-fax ports, if any, do not count toward calculating a rebate or determining eligibility.
• The Fax Upgrade Program is available worldwide.

PLEASE NOTE: Rebate checks may take up to 10 weeks from the day your boards were received at Dialogic and confirmed via email, and NOT the day you submit this form.
